Physical Group Change/Add Form

Aloha and welcome to our new Physical Group Change/Add form. Please use this form to submit changes for physical meetings. If you are looking to add or change a listing for an online meeting, please use our Online Meeting Submission Form.

Submitting a physical meeting listing does not automatically change the group’s status or add to the online meeting schedule. The group’s Intergroup Rep or Group Contact must still submit a NEW Online Meeting Submission Form in order to be listed there.

Please be sure to fill out the form as completely as possible. If there is something you need to explain that doesn’t fit in the form fields, please use the final comment box to let us know your needs or how we can improve the form. Mahalo
